Final Reporting Procedure*

A brief Program Completion Report* must be submitted to the STELLAR Program Coordinator within two (2) weeks of the completion of the program. An applicant will not be eligible for reimbursement until a final report has been filed with EDU.

Any materials remaining or equipment/resources secured in support of the approved proposal are to remain as property of the school for curriculum-based use by the teaching staff. To this end, materials, equipment, or resources must be inventoried and tagged as follows – Property of (insert school name), c/o STELLAR Program for Mathematics Professional Learning, 20XX (insert year). This information must also be reflected in the Program Completion Report.

Should the professional learning be cancelled (and not rescheduled), the applicant will be expected to notify EDU at STELLAR@gov.nl.ca as soon as possible.

Reimbursement will be transferred to the applicant pending EDU acknowledgment of receipt of the Program Completion Report. Failure to submit this report within two (2) weeks of the conclusion of the session may result in a delay, or unavailability, of funds.

Upon completion of this report, please forward TWO emails to STELLAR@gov.nl.ca:
First email - your date of birth
Second email - your employee number (found on your pay stub)
